gpn05 PUBLIC NOTICE REQUEST FOR PROPOSALS

Heart of Georgia Altamaha Regional Commission AAA (HGARC-AAA) is seeking competitive proposals from qualified entities interested in the operation of the Money Follows the Person (MFP) Transition Program. Applicants must have at least two years experience in operating the MFP program and trained staff that can provide services in the entire region.
Services will begin immediately upon award through funding received from the Georgia Department of Human Services’ Division of Aging Services. Appling, Bleckley, Candler, Dodge, Emanuel, Evans, Jeff Davis, Johnson, Laurens, Montgomery, Tattnall, Telfair, Toombs, Treutlen, Wayne, Wheeler, and Wilcox counties are to be served.
Request for proposal packets are available by contacting HGARC-AAA at 331 West Parker Street, Baxley, GA 31513, 912-367-3648. Three copies (1 original) of the proposal must be submitted no later than September 9, 2011 at 4:00 p.m. EST. NO EXCEPTIONS to this date and time of submittal will be considered or allowed. HGARC retains the right to reject any and all proposals.
