(the “Owner”), pursuant to the provisions of O.C.G.A. § 36-91-1, et. seq., herein seeks competitive proposals from prequalified general contractors for the construction of A NEW HEALTH DEPARTMENT BUILDING located at 238 Walnut Street in Baxley, Georgia 31513.
The requirements for construction of the Project, and the duties and responsibilities of the contractor whose Proposal is accepted, are set forth in the Contract Documents which include the Instructions to Proposers; Contract for Construction and Incorporated General Conditions (the “Contract”); supplementary and other conditions; the drawings; the specifications; and, any addenda issued by the Architect. Contractors interested in submitting Proposals must obtain copies of the Contract Documents by contacting the Project Architect; DeLoach & Fry Architects, LLC, P.O. Box 6926 (721 Walnut Street) Macon, Georgia, 31208 478/742-4999 – FAX 478/742-5207 – email: arch.group@deloachfryarchitects.com, a deposit in the amount of Two Hundred Fifty Dollars ($250.00) (not including shipping or handling fees) is required for each set of the Contract Documents. The full amount of the deposit, for two (2) sets less shipping & handling charges, will be refunded to each general contractor who submits a bona fide proposal upon the return of such sets in good condition within ten (10) days after the due date of the proposals. All other deposits will be refunded less the cost of reproduction of documents equal to One Hundred Fifty Dollars ($150.00), plus shipping and handling upon the return of documents in good condition within ten (10) days after the due date of the proposals. Any contractor submitting a Proposal must procure a complete set of the Contract Documents from the Project Architect. The Architect will also make available the Proposal Form as well as the Contractor’s Qualification Statement and Questionnaire and the Subcontractor’s Qualification Statement and Questionnaire, all of which must be fully completed and submitted with any Proposal.
Any Contractor who intends to submit a Proposal is required to attend a Mandatory Pre-Proposal meeting, which will be held on the 18th day of July 2013 at 10:00 AM at the offices of the Appling County Board of Commissioners located at 69 Tippins Street in Baxley, Georgia 31513. The Contract Documents require, among other things, the furnishing of all materials, labor and equipment for construction of the Project. The Owner reserves the right to make available other relevant documents or information concerning the Project.
Any Proposal submitted in response to this Request should comply strictly with all requirements set forth in the Instructions to Proposers. Any such Proposal must contain the completed Proposal Form setting forth the contractor’s proposed lump sum contract price for full and compete construction of the Project in conformity with all requirements of the Contract Documents. Any Proposal must include a fully executed Bid Bond in the amount of five percent (5%) of the proposed lump sum contract price (exclusive of any alternates and unit prices) in the form required by the Instructions to Proposers. As noted above, any contractor submitting a Proposal must also complete and submit the required Contractor’s Qualification Statement and Questionnaire and a completed Subcontractor’s Qualification Statement and Questionnaire for the site work, electrical, plumbing, mechanical, structural steel, masonry, and roofing subcontractors. For any work requiring a specialty or professional license, only licensed subcontractors may be submitted for consideration, and copies of all applicable licenses shall be attached to the Subcontractors’ Qualification Statements and Questionnaires. The Owner in its evaluation of Proposals will rely upon a contractor’s identification of proposed subcontractors. NO SUBSTITUTION OF SUBCONTRACTORS IS ALLOWED, AND IN THE EVENT OF ANY ATTEMPTED SUBSTITUTION AFTER THE DATE DESIGNATED AT THE PRE-PROPOSAL MEETING, THE BOARD MAY, IN ITS SOLE DISCRETION, DISQUALIFY ANY PROPOSAL FROM CONSIDERATION OR CANCEL ANY CONTRACT AWARDED TO ANY SUCH CONTRACTOR.
In evaluating Proposals, the Owner may seek additional information from any contractor concerning such contractor’s Proposal or its proposed subcontractors’ qualifications to construct the Project.
The Owner intends to award the construction contract to the responsible and responsive contractor whose Proposal is determined in writing to be the most advantageous to the Owner, taking into consideration the following evaluation factors which are listed in their order of relative importance:
Number of projects of similar scope and cost (similar to Appling County Rec. Gym and Ball Fields) that the contractor has successfully completed within the last ten years. Scoring will be as follows: (Possible 45 points)
One Project = 10 Points
Two Projects = 20 Points
Three Projects = 30 Points
Four Projects = 40 Points
Five or more = 45 Points
(B) The contractor’s proposed lump sum contract price for full and complete construction of the Project in conformity with all requirements of the Contract Documents. (Possible 40 Points)
Lowest Price: 40 Points
0 – 1% variance from low price: 35 Points1% - 3% variance from low price: 30 Points
3% - 5% variance from low price: 25 Points
Greater than 5% variance from low price: 20 Points
The qualifications and experience of those personnel whom the contractor will assign to the Project. (Possible 35 Points)
Contractor’s Project Manager: 1–20 points based on experience
Contractor’s Project Superintendent: 1–15 points based on experience
The qualifications and experience of the proposed site work, concrete, masonry, roofing, structural steel, plumbing, fire protection, mechanical, electrical, and fire alarm subcontractors. (Possible 27 Points)
Site Work: (Scale of 1 –3 points where 3 is the highest possible score)
Concrete: (Scale of 1 –3 points where 3 is the highest possible score)
Masonry: (Scale of 1 –3 points where 3 is the highest possible score)
Roofing: (Scale of 1 –3 points where 3 is the highest possible score)
Plumbing: (Scale of 1 –3 points where 3 is the highest possible score)
Fire Protection: (Scale of 1 –3 points where 3 is the highest possible score)
Mechanical: (Scale of 1 –3 points where 3 is the highest possible score)
Electrical: (Scale of 1 – 3 points where 3 is the highest possible score)
Fire Alarm: (Scale of 1 –3 points where 3 is the highest possible score)
The contractor’s history of successfully constructing projects without unnecessary contractual disputes, claims, arbitration or litigation. (Possible 25 Points)
Litigation(s), arbitration (s) and or claim (s) against Owner or Architect in the past 10 years:
None = 25 Points
One (1) = 10 Points
Two (2) or more = 0 Points
The contractor’s proposed unit pricing for items listed in the proposal. (Possible 23 Points)
(G) The Contractor’s proximity to Baxley, Georgia (Possible 30 Points)
Within 75 miles = 30 Points
Within 125 miles = 20 Points
Greater than 125 miles = 10 Points
All Proposals must be properly signed, submitted in triplicate in a sealed envelope, and received to the Appling County Board of Commissioners office located at 69 Tippins Street, Baxley, GA. 31513, no later than 2:00 PM Thursday, August 1, 2013. A list of all Subcontractors and Subcontractor’s Qualifications must be provided no later than 9:00 AM, Friday, August 2, 2013.
The Owner will receive such Proposals at such time and place. Proposals received after said time will not be accepted. At the discretion of the Owner, and in conformity with the applicable provisions of Georgia Law, the Owner may afford contractors an opportunity for subsequent discussion, negotiation, and revision of Proposals. The Owner reserves the right to reject any or all Proposals and to waive any technicalities or formalities.
Any Proposal submitted in response to this Request shall remain open for acceptance by the Owner, and same shall be honored by the contractor, for a period of Sixty (60) days from the date set forth hereinabove for the receipt of Proposals.
