You are hereby notified that Kacey Boatright, petitioner, filed a petition in the Superior Court of Appling County, Georgia, on the 19th day of February, 2013, praying for a change in the name of her minor child, from Gracey Lane Herrington to Gracey Lane Porter and that by reason of an order for service of summons by publication entered by the court on the 21st day of February, 2013, you are hereby commanded and required to file with the clerk of said court and serve upon David McCrea, petitioner’s attorney, whose address is P.O. Box 412, Alma, Georgia 31510, any objections to said name change within thirty (30) days of the date of the order of service by publication. If you fail to do so, judgment by default will be taken against you for the relief demanded in the petition.
